Program Description:
MomsFirst is a home visiting program that helps mothers succeed during and after pregnancy. MomsFirst participants are assigned a Community Health Worker (CHW) and may stay in the program until your baby is 18 months old. Our CHWs provide: Home visits or face-to-face visits at least once a month. Support to pregnant women throughout their pregnancies. Support to help ease the stress that comes with becoming a parent. Education on care before and after baby arrives, breastfeeding, mental health, family planning, and more.

Program Requirements:
Pregnant women, residing in the city of Cleveland.

Pregnant with Possibilities Resource Center

Program Description:
Making Opportunities Matter (MOM) — Research- based tailored for individuals who are pregnant or up to 11 months postpartum. It aims to educate and support clients throughout their perinatal journey, focusing on reducing toxic stress and overcoming barriers that can lead to negative birth outcomes. The curriculum is delivered through a group-based client-centered model of care.

Program Requirements:
Pregnant or 11 months postpartum and reside in Cuyahoga County

Monday-Friday 8am-5pm

Charlie Health

Program Description:
Charlie Health is a leading virtual intensive outpatient programming provider designed to treat those struggling with high-acuity mental health conditions. Charlie Health’s Virtual IOP is more structured and intensive than traditional weekly therapy, offering high-risk patients multiple hours of weekly treatment. At Charlie Health, each patient receives a customized treatment plan that includes group, individual, and family sessions weekly. Charlie Health also offers psychiatric services as needed. In addition, they offer a suite of wrap-around services, including 24/7 crisis care, free family support groups, and more.

Pathways to Purpose

Program Description:
Pathways To Purpose provide safe and stable housing and support to pregnant and or parenting teen mothers in foster care.

Program Requirements:
Teen Mothers currently in foster care.
